In this vital role, you will provide compassionate, high-quality nursing care directly in patients' homes, promoting health, safety, and independence. Duties Conduct comprehensive patient assessments during home visits, including vital signs, physical examinations, and medication reconciliation. Administer medications and injections accurately following physician orders while adhering to HIPAA and safety protocols. Perform specialized procedures such as IV infusion, catheter care, tube feeding management, and sterile dressing changes. Educate patients and their families on disease management, medication use, nutrition, and self-care techniques to promote health literacy. Collaborate with interdisciplinary teams using EMR (Electronic Medical Record) systems like Epic or eClinicalWorks to document care plans and progress notes clearly and thoroughly. Monitor patient conditions closely, triaging urgent needs and escalating concerns promptly to physicians or emergency services when necessary. Assist with discharge planning and coordinate ongoing care needs for patients transitioning between hospital, outpatient, or long-term care settings. Experience Valid LVN license with current state certification required; prior experience in home health nursing strongly preferred. Demonstrated proficiency with EMR/EHR systems such as Cerner or Athenahealth for accurate documentation. Critical care experience including ICU or emergency room settings is highly desirable; familiarity with Level I Trauma Center protocols a plus. Hands-on experience with diverse patient populations including geriatrics, pediatrics, neonatal ventilator management, or individuals with developmental disabilities. Knowledge of
ICD-9/10
coding, medication administration techniques, infection control practices, and basic life support certification required. Prior work in settings such as assisted living facilities, nursing homes, hospice care, or post-acute environments enhances your candidacy. Strong assessment skills in physiology and anatomy; ability to perform venipuncture, phlebotomy, specimen collection/processing and sterile procedures confidently.
