Primary Responsibilities :
Review and process new referrals, including order and clinical documentation. Build and maintain relationships with referral sources. Handle inbound inquiries from clients, family members, and field team members. Manage patient care coordination with field teams. Oversee resource coordination, including specialty pharmacy, DME, supplies, and lab arrangements. Provide hands-on care for troubleshooting, as needed. Coordinate with sales acceptance of patients. Manage documentation and participate in the on-call rotation with clinical leaders. Collect and review client needs to support scheduling. Deliver exceptional client service for internal and external stakeholders. Promote continuity and safety in client care, adhering to quality standards.
Experience and Education Requirements :
High School diploma or equivalent. Active LVN TX license required (must be unrestricted) Proven ability to communicate effectively with referral sources and clients. Demonstrated skills in de-escalation, client service, and high-volume call handling. Proficiency with web-based systems, MS Outlook, Word, and Excel.
Minimum Skills and Abilities :
Strong communication and interpersonal skills. Organizational skills with a keen attention to detail. Ability to manage pressure and multitask effectively. Commitment to quality care and client support.
Work Environment :
This role involves working in a high-paced office setting with continuous phone interaction. Physical requirements include sitting and standing for long periods, occasional bending and stooping, and lifting up to 10 lbs.
