Overview We are seeking a compassionate and dedicated Licensed Vocational Nurse (LVN) to join our home health care team. The ideal candidate will provide high-quality patient care to individuals in their homes, ensuring their comfort and well-being. This role requires a strong commitment to patient advocacy, excellent communication skills, and the ability to work independently while traveling to various locations. Duties Deliver direct patient care in accordance with established care plans and protocols. Monitor and document patients' health status, including vital signs and any changes in condition. Administer medications and treatments as prescribed by physicians. Educate patients and their families about health management and self-care techniques. Collaborate with healthcare professionals to develop and adjust care plans as necessary. Maintain compliance with HIPAA regulations to ensure patient confidentiality. Provide support in sterile processing and assist with procedures such as suturing when required. Travel to patients' homes for assessments and ongoing care. Skills Strong knowledge of patient care principles, particularly in senior care settings. Familiarity with HIPAA regulations and commitment to maintaining patient confidentiality. Proficient in primary care practices, including administering gastric lavage if necessary. Ability to perform basic medical procedures such as suturing and sterile processing. Excellent interpersonal skills for effective communication with patients, families, and healthcare teams. Strong organizational skills to manage multiple patient schedules effectively.
