Job Description:
As an LVN working for our home health agency, you will play a crucial role in delivering quality healthcare services to our patients in the comfort of their homes. You will work closely with patients, their families, and a multidisciplinary team to ensure the provision of personalized care that meets their unique needs. This role requires compassion, adaptability, and excellent clinical skills to provide comprehensive care and support to patients in their home environments.
Job Duties:
Patient Assessment and Monitoring:
Conduct initial and ongoing assessments of patients' health status. Monitor vital signs, observe for any changes in condition, and report findings to the supervising Registered Nurse (RN) or physician. Assess patients' needs for medical equipment, supplies, and additional services.
Medication Administration:
Administer medications as prescribed by the physician, ensuring accurate dosage and proper administration techniques. Educate patients and their families on medication regimens, potential side effects, and proper medication storage.
Wound Care and Dressing Changes:
Perform wound care and dressing changes according to physician orders and established protocols. Monitor wound healing progress and report any concerns to the healthcare team. Assistance with Activities of Daily Living (ADLs): Assist patients with activities such as bathing, grooming, toileting, and mobility as needed. Provide support and encouragement to promote independence and maintain optimal functioning.
Patient Education and Support:
Educate patients and their families on disease management, self-care techniques, and strategies for maintaining health and well-being. Provide emotional support and counseling to patients and families, addressing their concerns and answering questions.
Documentation and Reporting:
Maintain accurate and thorough documentation of patient assessments, care provided, and communication with other healthcare professionals. Report any significant changes in patients' condition or concerns to the supervising RN or physician promptly.
Collaboration and Communication:
Collaborate effectively with the interdisciplinary team, including RNs, therapists, social workers, and other healthcare providers, to coordinate patient care. Communicate with patients' primary care physicians and specialists to ensure continuity of care and adherence to treatment plans.
Safety and Infection Control:
Adhere to infection control practices and safety protocols to minimize the risk of healthcare-associated infections and promote patient safety. Maintain a clean and safe environment for patients and their families during home visits.
Continuing Education and Professional Development:
Stay updated on current nursing practices, guidelines, and regulations through ongoing education and training opportunities. Seek opportunities for professional growth and development to enhance clinical skills and knowledge.
Qualifications:
Graduation from an accredited LVN program. Current LVN licensure in the state of practice. CPR certification. Strong clinical assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work independently and as part of a team. Prior experience in home health or a similar setting preferred.
Pay:
$26.00 - $33.00 per hour
