Round Rock, TX Job Details PRN $45 - $140 an hour 1 day ago Benefits Flexible schedule Qualifications CPR Certification Customer communication RN License Case management Nursing Full Job Description Summit Home Health and Hospice is looking for a self-disciplined and team-oriented Home Health RN Case Manager with exceptional customer service skills to service our Austin area.

The RN implements the plan of care for patients. Performs and documents OASIS data sets. The RN provides care needed as described in departmental policies and procedures and all other duties as assigned by supervisor.
Essential Job Functions:
Completes initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients/clients . Completes assessments at appropriate time points, including Outcome and Assessment Information Set (OASIS) or other assessments as appropriate to the patient. Regularly re-evaluates the patient's/client's nursing needs and evaluates the outcomes of care. Develops, initiates, and revises the plan of care as necessary to ensure quality and continuity of care. Initiates appropriate preventive and rehabilitative nursing procedures. Refers to other services as needed. Plans for the discharge of the patient/client from services. Coordinates services. Informs the physician and other personnel of changes in the patient's/client's condition and needs. Prepares clinical and progress notes. Completes appropriate documentation in a timely manner. Promotes the Agency 's philosophy and administrative policies. Performs on-call responsibilities and provides on-call services to patients/clients and their families as assigned. Provides effective communication to patients/clients, their family members, team members, and other health care professionals.
Required Education:
Valid RN License Preferred Experience:
1 year home health experience 1 year Kinnser/WellSky experience Pay Routine visits starting at $45 per visit up to $135 for
SOC Pay:
$45.00 - $140.00 per hour
