Shift Type = Nights Shift Hours = 7:00 PM•7:00 AM Assignment Length = Per Diem / Up to 1 year On‑Call = Not specified Holidays = As needed based on coverage Patient Ratio = Varies by acuity Equipment = Telemetry monitors, ventilators, IV pumps, trach care equipment EMR = Not specified Description of job: SnapCare is seeking a highly skilled and dependable LTAC / Telemetry Registered Nurse to provide per diem night‑shift coverage at Cornerstone Hospital Round Rock in Round Rock, TX. This role focuses on caring for medically complex patients requiring long‑term acute care, telemetry monitoring, and advanced clinical support. The RN will utilize strong critical thinking and clinical judgment to assess patient needs, implement care plans, and ensure safe and effective treatment. This position requires leadership, the ability to supervise support staff, and flexibility to adapt in a high‑acuity environment.
Qualifications:
Minimum 2 years of recent LTAC or Telemetry RN experience Current BLS and ACLS certifications (AHA) Active Texas or compact RN license Strong experience with telemetry monitoring and complex patient care Excellent communication, assessment, and clinical decision‑making skills Ability to work independently on night shifts
Responsibilities:
Provide comprehensive nursing care for high‑acuity LTAC patients Monitor telemetry and respond to changes in patient condition Assess patients and develop individualized care plans Administer medications, IV therapies, and treatments Document care accurately per facility and regulatory standards Delegate tasks and supervise support staff appropriately Collaborate with physicians and interdisciplinary care teams Maintain patient safety and adhere to infection‑control protocols
