In this vital role, you will provide expert nursing assessments, manage patient needs, and coordinate care plans during weekday evenings and nights. Your dedication will ensure patients receive timely, high-quality support in their final stages of life, promoting comfort, dignity, and peace. This position offers an exciting opportunity to make a meaningful difference while utilizing your extensive clinical expertise across diverse settings. Duties Respond promptly to on-call patient needs during weekdays, providing clinical guidance and support via phone or telehealth. Conduct comprehensive patient assessments, including vital signs, symptom management, and physiological evaluations. Collaborate with interdisciplinary teams to develop and adjust individualized care plans aligned with patient wishes and clinical best practices. Manage urgent situations by coordinating emergency responses or hospital transfers when necessary, ensuring seamless communication with providers and families. Document all interactions accurately within electronic health records (EHR), maintaining compliance with HIPAA and health regulation policies. Educate patients and families on symptom management, medication administration, and end-of-life care options to promote understanding and comfort. Experience Background in hospice or palliative care with familiarity in managing complex symptoms related to infectious diseases or geriatrics. Experience with electronic health records (EHR) management and medical records documentation is essential.
