San Angelo, TX, 76901 Job Responsibilities Makes the initial nursing evaluation visit. Continually evaluates the patients' nursing needs. Initiates and coordinates the plan of treatment and necessary revision. Instructs and supervises the patient's family in the care of the patient and maintenance of a healthy environment for the patient. Provides those services requiring substantial specialized nursing skill. Renders nursing care to homebound patients and performs treatments as ordered by the physician. Assists the patient's family to recognize the daily living problems of the patient through all appropriate services for the patient. Maintains accurate records of the history of the patient for the supervisor and the Physician especially in reference to drugs, treatments, etc. Reports to the patient's physician and other appropriate personnel pertinent changes in the patient's condition. Supervises and teaches all other personnel providing services to the patient. Participates in the in-service programs attending at least two in-services educational meetings per year and as required. Participates in nursing reports and follow protocol for agency. Performs 60-day record review (recertifications) Participates in staff conference regarding patient care. Participates in the QAPI plan and process. Job Requirements Current licensure in the state of Texas of Compact license. Preferably has one year experience of nursing care in a hospital or home health setting. Current driver's license & access to a dependable vehicle with vehicle insurance.
