Job Description

JOB DUTIES
Providing services in accordance with agency policies Assist the Physician and Registered Nurse in performing specialized procedures. Assist the patients in learning appropriate self-care techniques Assessment of patient and documentation Assisting in teach of patients and/or family Assist the Registered Nurse in carrying out the plan of care Prepares equipment and materials for treatment observing aseptic technique as required Assist in planning, implementing and evaluating patient care Observation of patient and recording pertinent information and/or treatment(s) Other duties with supervision of Registered Nurse
JOB REQUIREMENTS
Current State Texas Licensed Practical Nurse LPN/LVN Minimum one (1) year of Medicare Home Health Care experience as an LVN/LPN Understanding of Home Health Care, Medicare Conditions of Participation, Federal, State guidelines and regulations. Excellent organizational, interpersonal and communication skills Experience managing central lines, wound vac, tracheotomy, colostomy, and drawing labs
