As a locally owned, family-operated, faith-based hospice serving the North Texas region, we are committed to providing exceptional end-of-life care while supporting patients and families with compassion, grace, and respect. We are seeking a relationship-driven LVN or RN Community Liaison who is passionate about serving others, building meaningful partnerships, and helping patients access the hospice care they deserve. Responsibilities Develop and maintain strong referral relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, memory care facilities, home health agencies, case managers, social workers, and other healthcare professionals. Educate healthcare providers, patients, families, and community partners about hospice eligibility, benefits, services, and philosophy of care. Conduct in-service presentations, educational events, and community outreach activities to increase awareness of hospice services. Participate in facility care conferences, interdisciplinary meetings, and community healthcare events as requested. Assist with patient referrals, admissions coordination, and obtaining hospice consents when appropriate. Serve as a trusted resource for patients, families, and referral partners throughout the hospice admission process. Maintain ongoing communication with referral sources to ensure excellent service and continuity of care. Represent Shoreline Hospice Care professionally and ethically within the healthcare community. Document referral activities and maintain accurate records of outreach efforts. Uphold patient confidentiality and comply with all HIPAA, Medicare, and regulatory requirements. Qualifications Current Texas LVN or RN license required. Hospice, home health, palliative care, case management, or healthcare marketing experience preferred. Strong knowledge of hospice eligibility, Medicare hospice benefits, and end-of-life care services preferred. Proven ability to establish and maintain professional relationships within the healthcare community. Excellent communication, presentation, and interpersonal skills. Self-motivated, organized, and able to work independently. Valid Texas driver's license and reliable transportation.
