Schedule:
Monday-Friday | 4:00 PM - 12:00 AM Are you a compassionate RN who thrives in providing comfort, support, and expert clinical care when patients and families need it most? Join our hospice team and make a meaningful difference in the lives of patients facing life-limiting illnesses. As an On-Call Hospice RN , you will provide after-hours nursing support, symptom management, patient assessments, and end-of-life care in patients' homes and care facilities. You'll work independently while collaborating with an interdisciplinary team to ensure patients receive exceptional, patient-centered care focused on comfort, dignity, and quality of life. What You'll Do Respond to after-hours patient and family needs from 4 PM to midnight. Perform comprehensive patient assessments and evaluate changes in condition. Develop, implement, and update individualized plans of care. Provide symptom management, medication education, and end-of-life support. Educate patients, families, and caregivers on disease progression and care needs. Coordinate care with physicians, hospice aides, social workers, chaplains, and other team members. Document patient visits and interventions accurately and timely. Provide clinical oversight and direction to hospice aides and LVNs/LPNs as needed. Qualifications Current RN license in good standing. Current CPR certification. Valid driver's license and auto insurance. Minimum of 2 years of hospice nursing experience preferred. HomeCare HomeBase experience preferred. Strong assessment, communication, and critical thinking skills.
