In this vital role, you will provide compassionate patient care across various settings, including long-term care, assisted living, and acute care environments. Your expertise will ensure that residents and patients receive high-quality nursing services, promoting their health, safety, and well-being. This position offers an exciting opportunity to make a meaningful difference in the lives of individuals with diverse healthcare needs. Responsibilities Deliver comprehensive nursing care to residents and patients, including medication administration, vital signs monitoring, and physical assessments. Assist with complex procedures such as IV infusion, catheterization, tube feeding, and wound care using aseptic techniques. Collaborate with interdisciplinary teams to develop and implement individualized care plans tailored to each patient's needs. Conduct triage assessments for incoming patients or residents to determine urgency and appropriate interventions. Document all patient interactions accurately within electronic health record (EHR) systems like Epic or Cerner, ensuring compliance with HIPAA regulations. Monitor patients' physiological status during acute episodes or chronic conditions such as dementia or Alzheimer's disease. Provide education and support to residents and their families regarding health management, medication use, and discharge planning. Requirements Valid LVN license in good standing with the state licensing board. Proven experience in critical care environments such as ICU or Level I trauma centers is highly desirable. Strong background working with individuals with developmental disabilities, dementia care, memory care, or hospice settings. Familiarity with EMR/EHR systems including Epic, Cerner, or Athenahealth for accurate medical documentation. Knowledge of medical terminology, anatomy, physiology, ICD coding (ICD-9/ICD-10), and CPT coding for procedures. Experience performing patient assessments, vital signs measurement, specimen collection/processing, and medication administration. Ability to work effectively in fast-paced environments such as emergency rooms or urgent care clinics while maintaining attention to detail. Excellent communication skills for collaborating with healthcare teams and educating patients/families on treatment plans.
