Job Summary The RN Nurse Home Visitor (NHV) provides home visits to first-time pregnant women and their babies throughout pregnancy and until the child reaches two years of age. The NHV is expected to adhere to the Nurse-Family Partnership (NFP) model of home visitation, applying independent critical thinking within the nursing process. This role involves assessing the physical, emotional, social, behavioral, and environmental needs of women related to their health and overall life course development. The NHV will also assess unborn children and their families, offering instruction, education, and mentoring in prenatal and postpartum care, nutrition, parenting and family dynamics, child health, family planning, and special health care needs. Additionally, the NHV will help families access community resources to improve infant outcomes and support the long-term well-being of the client and family. Traveling will be necessary between the office, client homes, and meetings. Client residences are in city and rural areas. Occasionally residences may be in unfamiliar neighborhoods that may have a history of criminal activity. This position primarily serves families in Smith Cherokee and Henderson Counties Major Responsibilities/Duties/Critical Tasks Understand, support and coach others on policies, procedures, guidelines, vision, mission, model and standards of Nurse-Family Partnership (NFP) and UT Health Science Center to both internal and external customers and colleagues. Participate in community coalitions in support of NFP promoting public awareness of NFP. Develop and maintain community relationships to support client referrals. Deliver instruction, education and provide mentoring for prenatal and postpartum care, nutrition, parenting and family dynamics, child health care, family planning, and special health care needs. Formulate nursing diagnosis(ses) based on holistic nursing assessment. Develop therapeutic relationships, utilizing concepts of reflection and motivational interviewing with women and their families in a home visiting environment. Assess physical, emotional, social, behavioral, and environmental needs of women, infants, children, and families related to health and life course development, as they relate to the NFP domains following nursing process in implementing NFP model into community nursing practice Assist women and their families in setting goals and provide education, support, and referrals to help them achieve those goals. Evaluates client progress toward stated goals and NFP outcomes and engage client and support systems in adjustments, as needed. Use reflective practice in one-on-one client work and provide personalized home visits based on client needs and NFP guidelines. Create, maintain, and support a safe environment for open discussion. Manages busy home visiting schedule, ensuring program fidelity and continued trust and engagement of clients and in accordance with client goals and NFP outcomes. Consults and collaborates with other professionals (healthcare and community) involved in providing services to women and families. Complete all required NFP education timely and actively engage in professional development to meet all NFP Nurse Home Visitor competency requirements. Identify sources of additional support and/or funding to broaden the scope and service to the community. Participates in quality improvement analysis and planning to identify areas for improvement. Identify more efficient/effective ways of providing services to clients, reduce waste, and be more effective stewards of grant funds. Accountable for the identification and reduction of unsafe practices that could cause harm to patients. Respect and support patient rights. Always maintain patient privacy and confidentiality of information and records. Prioritize and organize work so that documentation, reporting, and required assignments are completed within a specified time frame. Attend and actively participate in Team meetings, Case Conferences and activities to support the philosophy and goals of the program, funder, and university. Maintain competency in the patient safety functions related to position. Completion of Annual Mandatory Training, TB testing, and required licensure and certifications before expiration date. Other duties as assigned. Required Education/Experience Graduation from an accredited Registered Nurse program. EMR experience. Two (2) years recent experience in maternal, child health and public health home visiting. Preferred Education/ Experience Bachelor's degree. One (1) year experience mental or behavioral nursing. Bilingual a plus. Specialty Certification is preferred at hire and preferred within 12 months of eligibility. Required Certifications/Licenses Possess and maintain a current RN license or compact RN license to practice in Texas. Possess and maintain a current American Heart Association or American Red Cross BLS. Possess and maintain current CPR Healthcare Provider. Completion of Annual Mandatory Training. TB testing and required licensure and certifications before expiration date. Texas Driver's License (or receive within 30 days of hire date). Strong customer service and communication skills, both in person and over the phone. Proven track record in the development of quality clinical services. Distinct capacity to manage multiple responsibilities effectively. Demonstrated flexibility and initiative in a dynamic work environment.
