As a vital member of our hospice care team, you will deliver personalized nursing services that promote comfort, dignity, and quality of life. Your expertise will guide patients and families through complex medical needs with empathy and professionalism, ensuring seamless coordination of care across multiple disciplines. This role offers an opportunity to make a meaningful difference during one of life's most sensitive times while working in a dynamic, supportive environment. Duties Conduct comprehensive patient assessments to evaluate physical, emotional, and spiritual needs, ensuring holistic care delivery. Develop and implement individualized care plans in collaboration with physicians, social workers, and other healthcare professionals. Administer medications and treatments as prescribed, monitoring for efficacy and side effects to maintain patient comfort. Provide education and emotional support to patients and their families about disease progression, symptom management, and end-of-life planning. Coordinate discharge planning and ensure smooth transitions between inpatient, outpatient, or post-acute care settings. Document all patient interactions accurately in medical records while adhering to HIPAA regulations for confidentiality. Respond promptly to urgent patient needs or changes in condition, utilizing critical care skills gained from ICU or emergency medicine experience. Requirements Valid RN (Registered Nurse) license with current certification in good standing. Critical care experience with a background in ICU or emergency medicine strongly preferred. Experience working in Level I or Level II trauma centers or hospital medicine environments is highly desirable. Knowledge of physiology, medical terminology, infection control practices, and diagnostic evaluation procedures. Demonstrated ability to perform thorough patient assessments including vital signs and symptom management. Familiarity with discharge planning, case management, utilization management, and medical records documentation. Strong communication skills with the ability to provide compassionate patient service in high-pressure situations.
