As-needed scheduling based on availability Greater flexibility with no set weekly commitment Ideal for supplemental or occasional work Role Overview As a Home Health RN, you serve as the clinical lead for your patients
- managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge.
You'll combine advanced assessment, care planning, and patient education to reduce hospitalizations and support long-term independence. What You'll Do Conduct comprehensive in-home patient assessments Develop and manage individualized care plans Deliver skilled nursing care (medications, treatments, interventions) Serve as case manager coordinating across care teams Monitor progress and adjust care based on clinical judgment Educate patients and families on disease management Support care transitions and discharge planning Maintain timely, accurate EMR documentation Supervise Home Health Aides as appropriate Participate in on-call rotation (as applicable) What You Bring Active RN license (or compact eligibility) + CPR certification Graduate of an accredited nursing program Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred) Strong clinical judgment and independence Excellent communication and patient-centered approach Ability to manage caseload and prioritize effectively EMR proficiency Valid driver's license and reliable transportation What Sets You Apart Home Health or post-acute care experience Case management experience BSN Experience with Homecare Homebase (HCHB) What to Expect Field-based role covering the Wharton, TX area High autonomy with strong clinical support Dynamic environment requiring adaptability and critical thinking Performance expectations tied to patient outcomes, quality, and documentation Ready for a Different Kind of Nursing?
