Registered Nurse

Key Responsibilities:

Conduct comprehensive OASIS assessments, including Start of Care (SOC), Resumption of Care (ROC), Recertification, and Discharge assessments. Provide direct patient care, including wound assessments, wound care, and treatments as ordered by the physician. Administer IV infusions, perform venipuncture, and monitor patient responses. Educate patients and caregivers on disease processes, medication management, and preventive health measures. Collaborate with physicians and the interdisciplinary team to develop and implement individualized care plans. Ensure documentation accuracy and timeliness in compliance with agency policies and regulatory requirements. Participate in ongoing patient evaluation and adjust care plans as needed to achieve optimal outcomes.

Qualifications:

Current RN license in good standing. Home health experience preferred (OASIS proficiency a plus). Strong clinical assessment, communication, and organizational skills. Compassionate, patient-centered approach to care.
