Job Description
The Hospice & Palliative Care Nurse Navigator is responsible for guiding patients and families through the continuum of serious-illness care by providing education, care coordination, and support. This role bridges communication between hospitals, clinics, and BeyondFaith Hospice to ensure early identification of appropriate patients, timely referrals, and seamless transitions into hospice services. The navigator supports symptom management, goals-of-care discussions, and anticipatory guidance, improving patient outcomes and family experience. We are nurse owned and spiritually led, believing love makes the difference! Minimum 2 years RN experience required.
