PRN Registered Nurse (RN) Home Health Field Nurse

Genuine Home Health Services - Plano, TX Genuine Home Health Services is seeking a compassionate, dependable, and motivated PRN Registered Nurse (RN) to join our growing home health team in Plano and surrounding areas. In this role, you will provide skilled nursing care to patients in the comfort of their homes while helping them maintain independence and improve their quality of life. We are looking for clinicians who are passionate about patient-centered care, strong clinical outcomes, and delivering exceptional service to patients and families. Responsibilities Provide skilled nursing visits in accordance with physician orders and individualized care plans Complete OASIS assessments including SOC, Recertification, ROC, Follow-Up, and Discharge Perform hands-on patient care including: Wound care IV therapy Catheter care Medication administration and education Monitor patient conditions and communicate changes to the physician and care team Educate patients and caregivers on disease management, safety, medications, and wellness Complete accurate and timely clinical documentation in EMR systems Collaborate with therapists, aides, physicians, and interdisciplinary team members to ensure coordinated care Maintain compliance with agency policies, state regulations, and Medicare guidelines Qualifications Current and active RN license in the State of Texas Minimum 1 year of nursing experience required Home Health experience preferred Experience with OASIS documentation preferred HomeCare HomeBase, Axxess /WellSky, or Kinnser experience preferred Current CPR certification Strong clinical assessment, communication, and documentation skills Ability to work independently and manage time effectively Reliable transportation, valid driver's license, and current auto insurance required Schedule PRN / Per Diem Flexible scheduling Field-based position Benefits Competitive per-visit pay Flexible work schedule Supportive clinical team environment Opportunity to make a meaningful impact in patients' lives Join Genuine Home Health Services and become part of a team dedicated to compassionate, high-quality care delivered where patients feel most comfortable — at home.
