This role involves comprehensive assessment, care planning, patient and caregiver education, and coordination with an interdisciplinary team to ensure compassionate and effective care across the continuum of illness. Key ResponsibilitiesPatient Care Conduct comprehensive assessments of patients' physical, emotional, and psychosocial needs Develop, implement, and revise individualized care plans Provide skilled nursing care including wound care, medication administration, IV therapy, and symptom management Monitor and evaluate patient response to treatments and interventions Provide end-of-life care focused on comfort, dignity, and pain relief (hospice patients) Care Coordination Collaborate with physicians, social workers, therapists, and hospice team members Coordinate care transitions and referrals to ensure continuity of care Communicate effectively with patients, families, and caregivers regarding care plans and expectations Patient & Family Education Educate patients and caregivers on disease management, medications, nutrition, and safety Provide guidance on hospice philosophy and end-of-life processes Offer emotional support and counseling resources to patients and families Documentation & Compliance Maintain accurate, timely, and compliant clinical documentation Ensure adherence to Medicare, Medicaid, and agency regulations Participate in quality assurance and performance improvement activities On-Call & Support Duties Participate in on-call rotation as required Respond to urgent patient needs and provide intervention when necessary QualificationsRequired Current, active Registered Nurse (RN) license Valid driver's license and reliable transportation Current CPR/BLS certification Preferred Experience in home health, hospice, oncology, geriatrics, or palliative care Knowledge of Medicare/Medicaid regulations Strong assessment, critical thinking, and communication skills Skills & Competencies Compassionate, patient-centered approach to care Ability to work independently and manage time effectively Strong clinical judgment and problem-solving skills Emotional resilience and ability to support patients through end-of-life care Excellent communication and interpersonal skills Work Environment Patient homes, assisted living facilities, or hospice centers Frequent travel within assigned service area May involve evening, weekend, and on-call hours Physical Requirements Ability to stand, walk, bend, and lift up to 50 pounds Ability to perform patient transfers and assist with mobility Ability to travel between patient locations Additional Information Must maintain patient confidentiality in accordance with HIPAA regulations Commitment to providing culturally sensitive and respectful care Job Types:
