Job Description

Home Health RN Case Manager Location:
Salt Lake City, Utah Pay:
$36.00-39.00/hour DOE Join our compassionate care team as a Home Health RN Case Manager providing support throughout Salt Lake County. In this role, you'll admit patients, create care plans, and deliver meaningful, patient-centered care in the comfort of their homes.
What You'll Do:
Complete admissions and develop individualized Plans of Care Provide skilled nursing visits for home health patients Collaborate with case managers and interdisciplinary teams Educate patients and families while supporting comfort and dignity
What We're Looking For:
Active Utah RN license 2+ years nursing experience (home health or hospice preferred) Strong assessment, communication, and time management skills Reliable transportation and CPR certification
