RN Hospice Case Manager Location:
Salt Lake Valley Price:
$38.00 - $42.00 per hour DOE Looking for more than just a nursing job? Join a hospice team where your care truly makes a difference! What You'll Do:
Provide skilled nursing care and manage patient care plans Support and educate patients and families Collaborate with a strong, mission-driven team Complete OASIS documentation
What We're Looking For:
Active Utah RN license 2+ years nursing experience (home health/hospice a plus) Strong clinical judgment and communication Self-motivated and patient-focused
