We are seeking dedicated RNs to provide skilled nursing care for clients in and around the 7 cities of Hampton Roads. The ideal candidate is patient-focused, reliable, and confident in delivering high-quality care. This is a PRN position, allowing you to pick up visits based on your availability and preferred coverage area.

Immediate openings available Responsibilities Provide compassionate, patient-centered nursing care while treating all clients with dignity, respect, and kindness. Follow the established Plan of Care and physician orders to ensure safe and effective patient care. Accurately document all patient care, assessments, and interventions using the agency's electronic medical record (EMR) system. Communicate regularly with the Director of Nursing regarding patient status, changes in condition, concerns, and care coordination needs. Observe, assess, and report any changes in a patient's physical, mental, or emotional condition. Administer medications, treatments, and skilled nursing services as ordered by the physician. Maintain professional communication with patients, families, caregivers, and members of the healthcare team. Demonstrate patience, empathy, and professionalism when caring for patients with complex medical needs. Adhere to all agency policies, nursing standards, HIPAA regulations, and infection control practices. Participate in ongoing training and education as required by the agency. Qualifications Current Virginia RN license in good standing Must be able to pass a background check with no barrier crimes or theft Minimum of one year of clinical experience preferred; home health experience strongly preferred. Must be able to pass a background check with no barrier crimes or theft-related offenses. Must be able to pass a 10-panel drug screen. Experience working with geriatric and rehabilitation patients preferred. Strong clinical assessment and treatment planning skills. Excellent communication, organizational, and interpersonal skills. Comfortable utilizing electronic charting and documentation systems. Current CPR certification. Valid driver's license and reliable transportation. Negative TB test within the past year.
