Day Shift Job Summary:

We are seeking an experienced Home Health Registered Nurse (RN) to join a highly respected healthcare organization serving patients throughout the local community. The Home Health RN will be responsible for assessing patients, developing and updating care plans, providing skilled nursing care, educating patients and families, and coordinating with the interdisciplinary care team to support positive patient outcomes. This position requires strong clinical judgment, excellent communication skills, and the ability to manage patient visits efficiently in a home health environment.

Key Responsibilities:

Provide skilled nursing care to patients in their homes. Complete patient assessments and develop individualized plans of care. Perform start-of-care, recertification, resumption-of-care, and discharge visits. Complete documentation, including OASIS documentation when applicable. Monitor patient progress and communicate changes in condition to team members. Educate patients and families on medications, disease management, treatment plans, and safety precautions. Coordinate care with physicians, therapists, case managers, and other healthcare professionals. Maintain compliance with agency policies, state regulations, and CMS guidelines. Manage a caseload and schedule patient visits effectively.

Qualifications:

Active Registered Nurse (RN) license. Previous home health experience preferred. OASIS documentation experience preferred. Strong assessment, communication, and time management skills. Ability to work independently in the field. Valid driver's license and reliable transportation required. BLS certification preferred or required based on facility guidelines.

Pay:

$65,000.00 - $100,000.00 per year
