Home Health RN in Chesapeake, VA Sunbelt Staffing - 3.8 Chesapeake, VA Job Details Contract 16 hours ago Qualifications Home health care assistance RN License In-home healthcare experience Driver's License Home health agency experience Full Job Description Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. This contract opportunity is ideal for an experienced Registered Nurse with a passion for Home Health, focusing on comprehensive patient support and community health improvement.
Schedule/Hours:
Flexible scheduling options available Day shifts with occasional on-call responsibility
Qualifications and Skills:
Active RN license in good standing Minimum of one year of recent Home Health nursing experience preferred Strong clinical assessment and critical thinking abilities Proficient in developing, monitoring, and revising care plans Familiarity with electronic health records and mobile charting devices Excellent interpersonal and communication skills Valid driver's license and reliable transportation, as travel between patient homes is required Current BLS certification
Key Responsibilities:
Conduct in-home patient assessments to develop personalized care plans Administer medications, perform wound care, and manage IV therapies as clinically indicated Provide patient and family education regarding ongoing health care needs Collaborate with physicians, therapists, and other care professionals to ensure optimal patient outcomes Monitor patient progress and update care plans as needed Maintain accurate and timely documentation compliant with healthcare regulations and standards
