Magnolia Post Acute, a leading provider of post-acute care, is seeking a highly skilled and compassionate Registered Nurse (RN) MDS Coordinator to join our team in Colonial Heights, VA. As an MDS Coordinator, you will play a crucial role in ensuring the highest level of care for our residents by developing comprehensive assessments and coordinating the Restorative and Comprehensive Assessment (RAI) process.
Key Responsibilities:
Develop preliminary and comprehensive assessments of the nursing needs of each resident, utilizing the forms required by current rules or regulations and Facility policies. Ensure that all members of the assessment team are aware of the importance of completeness and accuracy in their assessment functions and that they are aware of the penalties, including civil money penalties, for false certification. Coordinate the RAI process assuring the timeliness and completeness of the MDS, CAAs, and Interdisciplinary Care Plan. Monitor the facility's Quality Indicator (QI) and Quality Measure (QM) report. Develop the schedule of activities required for the resident assessment and plan of care. Inform all assessment team members of the requirements for accuracy and completion of the resident assessment (MDS). Ensure that each portion of the assessment is signed and dated by the person completing that portion of the MDS.
Requirements:
Registered Nurse with a current License in the state of Virginia. Must have, as a minimum, six (6) months training experience in rehabilitative and restorative nursing practices. Must have experience as an
RNAC MDS
