Job Summary Talin is seeking a compassionate and skilled Registered Nurse (RN) to join our Home Health team. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being. This role requires a strong foundation in medical documentation, a thorough understanding of physiology, and the ability to work independently while managing multiple patient needs. Duties Conduct comprehensive assessments of patients' health status and needs in their home environment. Develop and implement individualized care plans based on patient assessments and medical history. Administer medications and treatments as prescribed, monitoring patients for any adverse reactions or changes in condition. Document all patient interactions, care provided, and changes in health status accurately and timely. Educate patients and their families about health management, disease prevention, and treatment options. Collaborate with interdisciplinary teams to ensure coordinated care for patients. Assist with laboratory tests and procedures when required. Maintain compliance with all regulatory requirements and standards of practice. Provide support for various specialties including geriatrics, post surgical care, post-stroke care, cardiac care, diabetic care, and renal care. Experience Candidates should possess the following experience or skills: MUST have minimum 1 year clinical experience to apply. Proficiency in medical documentation practices. We use Aloraplus Must be knowledgeable documenting OASIS or willing to learn Knowledge of physiology relevant to patient care. Proficient in wound care a plus.
