Nursing degree, active registered nurse (RN) license and minimum 1+ years Home Health RN experience required. Applicants who do not meet these qualifications will not be considered. Key Qualifications:

Current, active RN license Minimum of one year of recent experience in home health nursing Ability to work independently and manage a patient caseload Strong clinical assessment and documentation skills Proficient with electronic health records Valid driver's license and reliable transportation Exceptional interpersonal, organizational, and communication skills CPR certification (AHA or equivalent preferred)

Role Responsibilities:

Provide skilled nursing assessments and direct patient care in private homes Develop and update individualized care plans based on patient status and needs Administer medications, treatments, and procedures per physician orders Educate patients and family members on ongoing care, disease processes, and wellness strategies Monitor patients' progress toward goals, reporting significant changes to the interdisciplinary team Maintain accurate, timely documentation of all care provided Collaborate with physicians, therapists, and social workers as needed to ensure coordinated care Participate in on-call rotation or weekends as required
