  As a Registered Nurse (RN)
• Assessment Specialist in home care, you will be responsible for conducting comprehensive client assessments to determine care needs, develop appropriate service plans, and ensure clients receive safe, high-quality non-medical support in their homes.

This role is primarily assessment-focused and does not involve ongoing direct skilled nursing care. You will collaborate with caregivers, clients, families, and the internal team to promote client safety, independence, and overall well-being.

Responsibilities:

Conduct initial in-home assessments to evaluate clients' physical, cognitive, and psychosocial needs. Complete routine reassessments and supervisory visits in accordance with state regulations and agency policies. Develop and update individualized plans of care based on assessment findings and client goals. Evaluate caregiver performance and ensure care is being delivered safely and appropriately. Provide guidance and education to caregivers based on client-specific needs and care plans. Identify any changes in client condition and communicate recommendations to the care team and family. Collaborate with office staff to ensure proper service alignment and continuity of care. Maintain accurate, timely, and compliant documentation of all assessments and supervisory visits. Ensure compliance with Virginia home care regulations, including documentation and oversight requirements. Serve as a clinical resource to the team for care-related questions and best practices. Participate in quality assurance and performance improvement initiatives as needed. Support client and family education to promote safety, independence, and quality of life.

Qualifications:

Active Registered Nurse (RN) license in the state of Virginia. Minimum of 2 years of nursing experience (home care or community-based experience preferred). Strong assessment and observational skills with attention to detail. Excellent communication and interpersonal skills. Ability to work independently and manage a flexible schedule. Experience with care planning and client evaluations. Proficiency with electronic documentation systems (EHR). Reliable transportation and willingness to travel to client homes. CPR certification (if required by agency policy). BSN preferred but not required.

Working Conditions:

Flexible scheduling based on assessment needs (may include occasional evenings). Travel to client homes within designated service areas. Exposure to a variety of home environments. Minimal physical demands compared to traditional bedside nursing, though some mobility and observation tasks are required.
