Job Description

Job Overview We are seeking a dedicated and compassionate Geriatric Nurse Practitioner to join our dynamic healthcare team. In this vital role, you will provide comprehensive primary and specialized care to elderly patients, promoting health, managing chronic conditions, and enhancing quality of life. Your expertise will help bridge the gap between acute and long-term care, ensuring our senior patients receive personalized, respectful, and expert medical attention. This position offers an exciting opportunity to make a meaningful difference in the lives of older adults through innovative clinical practices and collaborative patient-centered care. Duties Conduct thorough physical examinations and health assessments tailored to geriatric patients, including vital signs, functional status, cognitive evaluations, and medication reviews Manage acute and chronic conditions with advanced clinical skills Utilize EMR/EHR systems to document patient encounters accurately while ensuring compliance with HIPAA regulations Collaborate with families and caregivers to educate them on disease management strategies and support services available for memory care or dementia. Experience Familiarity with medical coding systems including CPT codes for procedures and
ICD-10/ICD-9
coding for diagnoses is essential for accurate documentation and billing processes. Experience working with people with disabilities or developmental delays within a healthcare context is a plus. Strong understanding of pharmacology related to psychopharmacology, pain management, hospice & palliative medicine enhances comprehensive patient support.
