Registered Nurse

Are you a compassionate and experienced Registered Nurse who enjoys working independently and making a meaningful impact in the community? Care with Dignity at Home® is seeking a professional and dependable In-Home Assessment Nurse to conduct client assessments for our non-medical home care agency serving seniors, Veterans, and adults who wish to remain safely and comfortably at home.

Responsibilities:

• Conduct initial in-home client assessments and reassessments
• Evaluate client needs, safety concerns, and level of care required
• Develop and update care plans based on assessment findings
• Communicate professionally with clients, families, caregivers, hospitals, case managers, and referral sources
• Complete documentation accurately and submit assessments in a timely manner
• Assist with care coordination and recommendations for services
• Support VA and private pay client assessments as needed

Qualifications:

• Active Virginia RN License in good standing
• Minimum 1 year nursing experience preferred
• Home care, hospice, case management, or geriatric experience preferred
• Excellent communication and assessment skills
• Reliable transportation and valid driver's license
• Ability to work independently and manage scheduling efficiently
