Job Overview:

We are seeking a dedicated Registered Nurse (RN) to join a growing home health team serving patients. This is a field-based role ideal for RNs who thrive on independence, critical thinking, and one?on?one patient care in the home setting.

Key Responsibilities:

Manage and coordinate patient care plans Perform in-home patient assessments and visits Collaborate with interdisciplinary care teams Monitor patient progress and adjust care plans Ensure documentation and compliance standards Educate patients and families on care plans Deliver compassionate, patient-centered care

Requirements:

Active RN License (Virginia or Multi-state Compact) BLS (AHA) within 90 days Associate Degree in Nursing At least 2+ years RN experience At least 1 year Home Health experience (REQUIRED) Reliable transportation required for patient visits
