Competitive, based on experience Job Summary We are seeking a compassionate and dependable Registered Nurse (RN) to provide assessment services for our New home care business in Virginia Beach. This is a contract, as-needed position for an RN who is comfortable working independently and conducting in-home client assessments. The RN will be responsible for evaluating client needs, documenting findings, and helping support safe, appropriate care planning. Responsibilities Conduct initial in-home client assessments Perform reassessments as needed based on client condition or service changes Evaluate clients' physical, cognitive, emotional, and functional needs Develop and update individualized care recommendations Complete accurate and timely clinical documentation Communicate assessment findings with agency leadership and care team members Identify changes in condition or risk factors that may affect care needs Support compliance with agency standards and applicable regulations Maintain confidentiality and professional standards in all client interactions Qualifications Active Registered Nurse (RN) license in Virginia Graduate of an accredited nursing program Current CPR certification Strong nursing assessment and documentation skills Ability to work independently and professionally in client homes Excellent communication and organizational skills Valid driver's license and reliable transportation Preferred Qualifications Experience in home care, home health, hospice, case management, or client assessments Knowledge of care planning and in-home service assessments Experience working with adult and senior clients Schedule Contract position As-needed based on client referrals and assessment volume Flexible scheduling Compensation Competitive per-visit or per-assessment rate, based on experience Why Join Us This role is ideal for an RN seeking flexible contract work while making a direct impact on client care.
