The Center for Living & Rehabilitation is seeking a dedicated and compassionate Registered Nurse (RN) to join our team! As a key member of our healthcare staff, you will play a vital role in providing high-quality patient care to our residents.

Responsibilities:

Provide skilled nursing care to residents in our Subacute Rehab wing Develop and implement individualized care plans in collaboration with the interdisciplinary team Monitor and report changes in resident condition to the nursing team and medical staff Communicate effectively with residents, families, and staff to ensure seamless care transitions Participate in quality improvement initiatives to enhance the resident experience Maintain accurate and detailed records of resident care

Requirements:

Current RN licensure in the state of Vermont Ability to work effectively as a team member in a fast-paced environment Strong communication and interpersonal skills
