Clearable for Federal Background Unit (U.S. Citizenship Required) We are seeking a dual-hatted Lead Nurse Case Manager / Healthcare Project Lead to oversee medical readiness, case coordination, and team operations for a dedicated federal healthcare delivery program. This on-site leadership position combines full-time, hands-on clinical case management with contract oversight, team supervision, and client liaison duties, Monday through Friday, daytime operations. The Lead will manage a specialized administrative and clinical support team to ensure high-quality execution of health tracking, regulatory documentation compliance, and medical evaluation processing. Occasional company-paid out-of-state travel is required to support centralized mobilization reviews. Flexibility is required to support scheduled operational readiness events per year.
Core Responsibilities Clinical Case Management & Coordination Care Monitoring:
Coordinate comprehensive, non-clinical care pathways for an active, fluctuating caseload of organizational members.
Healthcare Systems Management:
Actively input, track, and manage member medical records within specialized federal health tracking databases and electronic case management systems.
Profile Initiation:
Review provider referrals and establish electronic care profiles within mandated, tight turnaround timelines.
Medical Charting:
Maintain absolute accuracy and timeliness in charting; document and upload all member encounters and external clinical notes within standard operating review windows.
Evaluation Board Support:
Audit files, reconcile hardcopy and electronic histories for completeness, and provide updated status summaries to formal review boards.
Operational Readiness Events:
Support periodic, large-scale health assessment and deployment readiness review events throughout the year, utilizing flexible shift adjustments to maintain standard workweek constraints.
Program Management & Team Leadership Team Supervision:
Supervise a small on-site team of clinical professionals and health information technicians to guarantee flawless contract execution and adherence to the program's Quality Control Plan (QCP).
Primary Client Liaison:
Serve as the central point of contact for the government's technical representative and lead medical authorities to streamline daily operations.
Compliance Reporting:
Author, compile, and submit structured monthly metrics and profile management status reports to leadership by established monthly deadlines.
Quality Assurance:
Direct internal quality reviews and implement performance improvement processes to remain strictly aligned with federal healthcare guidelines and data protection policies.
Position Qualifications & Requirements Mandatory Minimum Qualifications Education:
Graduate of an accredited professional nursing program. Preferably Bachelor of Science in Nursing (BSN) or higher.
Licensure:
Must hold an active, unrestricted license as a Registered Nurse (RN). A valid multi-state Nurse Licensure Compact (NLC) license with privileges to practice in Vermont is fully acceptable.
Clinical Experience:
A minimum of two (2) years of direct clinical nursing or case management experience. Active Certified Case Manager (CCM) credential or equivalent professional designation is a plus. Recent experience (within the past 5 years) utilizing Department of Defense (DoD) or military medical readiness databases and tracking systems is highly desirable.
Supervisory Experience:
A minimum of two (2) years of formal supervisory, team lead, or charge nurse experience in a healthcare or case coordination setting (minimum of 4 years of overall professional nursing experience required).
Security & Citizenship:
Must be a U.S. Citizen capable of passing a federal Tier 1 background check (NACI) to secure an organizational credential and authorized network system access.
Pay:
$95.00 - $100.00 per month
