Exempt (salary)PURPOSE:
The Individualized School Nurse provides one-on-one support to a student with complex health issues in the school setting to ensure their health, safety, and access to education.ESSENTIAL DUTIES & RESPONSIBILITIES
Under the supervision or direction of the District Nursing Supervisor, the Individualized School Nurse will provide the following services as assigned:DIRECT STUDENT CARE
Provide continuous, individualized nursing care and monitoring to an assigned student during the school day and school sponsored activities, as needed. Administer medications and treatments in accordance with licensed healthcare provider orders and district policy Perform specialized procedures and manage medical equipment, as required. May include, but not limited to, tracheostomy care, suctioning, oxygen administration, G-tube feedings, urinary catherization, seizure management, etc.) Respond promptly to medical emergencies and initiate appropriate interventions. Accompany the student on school transportation, as needed.CARE PLANNING AND DOCUMENTATION
In collaboration with the building school nurse, contribute to the development of an Individualized Health Plan (IHP) for school. Implement and monitor the student's IHP. Maintain accurate, confidential health records consistent with principles of nursing documentation. Communicate changes in the student's condition to parents/guardians, appropriate school personnel, and/or the student's medical team. Data collection Attend and contribute to Individualized Education Plan (IEP) meetings, as requested.COLLABORATION AND SUPPORT
Collaborate with teachers, administrators, therapists, support staff, and the building school nurse to ensure a safe and healthy learning experience for the assigned student. Promote inclusion by supporting the student's participation in classroom and school activities.COMPLIANCE AND SAFETY
Adhere to state nurse practice acts, district policies, and school health regulations Ensure compliance with medication administration policies and infection control standards. Demonstrate proficiency in the nursing skills needed for the care of the assigned student. Maintain confidentiality in accordance with FERPA and HIPAA TOOLS/TECHNOLOGYREQUIREMENTS
Calculators or accessories, desktop and laptop computers, facsimile machines, photocopiers, electronic mail software, spreadsheets (google sheets), internet browser, student database software (PowerSchool), and electronic health record (Snap Health Center).QUALIFICATIONS
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily, in addition to the following: Current Vermont or Multi-State Registered Nurse License Current American Heart Association Basic Life Support (BLS) certification Minimum of three years of nursing experience working in a variety of health service settings, especially pediatric health carePHYSICAL/MENTAL DEMANDS
Must be able to move objects weighing up to 25 pounds Must be able to perform skilled nursing procedures necessary for the assigned student. Must be able to move around the health office, classroom, and school to access office machinery, assist students, attend meetings, and perform interventions. Must be able to effectively and efficiently operate under stressful situations, remain calm in emergencies, and work independently.WORKING CONDITIONS
Work is generally performed in a climate-controlled, office or classroom environment but may include time in the cafeteria, playground, school transportation, and school-sponsored activities. Noise level is moderate. Exposure to body fluids is possible. Use of personal protective equipment (PPE) and adherence to universal precautions is required, as indicated. No known environmental hazards are encountered in normal performance of job duties. This role follows the academic calendar and school hours but may require attendance on field trips or at extracurricular activities.
