Job Description

Today's Solutions LLC seeks two experienced Non-Clinical RN Case Managers to join our team supporting the Vermont Army National Guard at Camp Johnson, Colchester, Vermont. Working under the Lead RN Case Manager, these positions carry the core case management workload — managing an average of 130-250 active cases per RN, attending SRP and PHA readiness events, supporting MEB/PEB processing, and ensuring every Soldier navigating a medical issue receives timely, accurate, coordinated care. What You Will Do
  • Establish eCase profiles within 1 workday of provider referral and manage all assigned Soldier cases from referral through deployable status or MEB/PEB disposition.
  • Document all Soldier encounters and newly received medical documents in eCase, MEDCHART, MEDPROS, AHLTA, MWDE, HRR, and
DENCLASS
within 3 business days.
  • Coordinate with Soldiers, their PCPs, specialty providers, and unit commanders to track appointments, resolve scheduling conflicts, and ensure proper documentation.
  • Support MEB/PEB board preparation, including record reconciliation and file completeness review.
  • Attend SRP, PHA, and mobilization events at Colchester, VT and Fort Bliss, TX as required.
  • Maintain HIPAA compliance at all times and report any suspected PHI breach to the Lead RN immediately. Mandatory Qualifications
Licensure:
Active, unrestricted RN license. Vermont single-state OR valid NLC multi-state compact license with Vermont practice privileges.
    Experience:
    Minimum two (2) years of relevant clinical or case management nursing experience.
      Security:
      U.S. Citizen — eligible for Tier 1 (NACI) background investigation and CAC issuance. Preferred Qualifications
      • BSN or higher degree.
      • Certified Case Manager (CCM) credential.
      • Direct experience with Army medical readiness systems (MEDCHART, MEDPROS, MHS GENESIS, AHLTA, eCase) within the past 5 years.
      • Prior Military Health System or National Guard medical readiness experience.
