Hours:
36 Hours Per Week We are seeking a compassionate and team-oriented Home Health Registered Nurse (RN) for a 13-week assignment in the St. Johnsbury, Vermont area. This opportunity offers the ability to provide skilled nursing services to patients in their homes while working collaboratively with an interdisciplinary healthcare team.
Requirements:
Active Vermont Registered Nurse (RN) License required. Minimum 1 year of clinical nursing experience required. Current
BLS/BCLS
Certification required. Current ACLS Certification required. Home Health experience preferred. Experience with Homecare Homebase (HCHB) preferred. Strong assessment, care coordination, and patient education skills. Excellent communication and documentation abilities. Ability to work independently while managing a diverse patient caseload.
Responsibilities:
Provide skilled nursing care to patients in their homes. Perform patient admissions, assessments, and ongoing evaluations. Develop and implement individualized care plans. Monitor patient conditions and communicate changes to physicians and care team members. Educate patients and families regarding diagnoses, medications, treatments, and disease management. Coordinate care with physicians, therapists, caregivers, and interdisciplinary healthcare teams. Maintain accurate and timely electronic documentation. Ensure compliance with home health regulations, policies, and standards of care. Promote patient safety, independence, and quality of life.
