Case Manager/UR Nurse

The Care Manager, RN role with Gifford Medical Center is embedded within all departments and collaborates with the medical team to plan for the discharge of patients and to have all of the resources in place when the patient leaves the hospital. The Case Management RN needs to be a strong communicator, having to work with families, providers and insurance companies to consider the best needs for the patients. This role is responsible for triaging, coordinating and facilitating the clinical management for an assigned population. Strong time management/organization skills, ability to multi-task and communicate effectively in a professional manner are fundamental to the role.
