Hours Per Week Start Date:
ASAP Key Responsibilities Conduct comprehensive patient and caregiver assessments. Develop, implement, and update individualized hospice care plans. Serve as Case Manager for assigned hospice patients. Coordinate care with physicians, interdisciplinary team members, caregivers, and community resources. Provide pain and symptom management education and support. Perform wound care, IV therapy management, and other skilled nursing interventions as needed. Monitor patient progress and modify care plans based on changing clinical needs. Maintain accurate and timely documentation in McKesson EMR. Participate in interdisciplinary team meetings and care planning discussions. Qualifications Active Registered Nurse (RN) license required. Minimum 2 years of nursing experience. Hospice nursing experience strongly preferred. Strong case management and patient assessment skills.
