In this vital role, you will provide comprehensive nursing care to patients in their homes, ensuring safety, comfort, and optimal health outcomes. The ideal candidate will possess extensive clinical expertise, including critical care, trauma, and hospital medicine experience, to deliver high-quality patient-centered care across diverse settings. Your responsibilities will encompass patient assessment, care planning, medication management, and coordination with multidisciplinary teams to support recovery and well-being. Duties Conduct thorough patient assessments, including vital signs and physiological evaluations, to determine individual health needs. Develop and implement personalized care plans aligned with medical directives and patient goals. Administer medications, treatments, and interventions following established protocols and safety standards. Monitor patient progress regularly, adjusting care strategies as necessary to promote recovery. Educate patients and their families on disease management, medication adherence, and health maintenance strategies. Coordinate discharge planning and facilitate communication with primary care providers and specialists. Maintain accurate medical records in compliance with HIPAA regulations and documentation standards. Perform diagnostic evaluations and assist with complex cases involving infectious diseases or acute conditions. Manage case documentation for utilization review, case management, and post-acute care transitions. Ensure adherence to infection control protocols and safety procedures during home visits. Collaborate with interdisciplinary teams to optimize patient outcomes across outpatient, inpatient, or clinic settings. Skills Proven experience in critical care environments such as ICU or Level I Trauma Centers is highly desirable. Strong background in hospital medicine, emergency medicine, or post-acute care settings. Knowledge of physiology principles relevant to diverse patient populations including geriatrics and pediatrics. Experience with behavioral health assessments and management strategies. Familiarity with medical terminology, diagnostic evaluation processes, and medical records management. Ability to perform comprehensive patient assessments and vital signs monitoring accurately. Excellent communication skills for effective patient education and interdisciplinary collaboration. Proficiency in utilizing managed care protocols, discharge planning tools, and utilization management systems. Demonstrated ability to handle complex cases involving infectious diseases or trauma scenarios. Commitment to maintaining confidentiality in accordance with HIPAA regulations.
