4x10 Day Shift (Start ASAP) Position Overview "Navitas Healthcare, LLC" is seeking an experienced RN or MSW for a Case Management / Care Coordination role in an acute care hospital setting. The position involves discharge planning, utilization review support, and coordinating patient care across multiple hospital campuses. Key Responsibilities Perform discharge planning and care coordination for acute care patients Collaborate with physicians, nurses, and interdisciplinary teams Support case management activities across Med-Surg, ED, and long length-of-stay patients Assess patient needs and develop safe discharge plans Coordinate post-acute care services and community resources Ensure timely documentation in EMR (EPIC) Participate in care conferences and patient progress reviews Assist with utilization and utilization review workflows Work across multiple campuses as needed Ensure smooth patient transitions and continuity of care Required Qualifications Active RN License (or MSW eligible for case management role) Minimum 1 year of acute care experience required Discharge planning / care coordination experience required Strong preference for Med-Surg, ED, or LLOS case management experience BLS Certification required (for RN roles) Ability to work in fast-paced hospital environment Strong communication and organizational skills EPIC charting experience preferred For more details reach at
