As an MDS Nurse, you will play a vital role in coordinating and completing the Minimum Data Set (MDS) assessments, ensuring compliance with regulatory standards such as NCQA and CMS guidelines. Your expertise will support clinical decision-making, optimize care plans, and enhance quality outcomes across inpatient, outpatient, and long-term care settings. This position offers an exciting opportunity to utilize your nursing skills in a dynamic environment focused on improving patient health and operational efficiency. Responsibilities Conduct thorough MDS assessments for residents or patients, accurately capturing clinical data related to physiology, anatomy, and medical history Review and analyze medical records, documentation, and coding (ICD-10, CPT) to ensure completeness and accuracy for reimbursement and quality reporting Collaborate with interdisciplinary teams including physicians, case managers, and health information management staff to develop individualized care plans aligned with regulatory standards Utilize EMR systems such as PCC and Epic, to document findings efficiently and maintain up-to-date medical records Perform utilization review and management activities to support appropriate level of care decisions in acute care, hospice, or home care settings Ensure compliance with HIPAA regulations by safeguarding patient information during documentation review and data entry processes Participate in discharge planning, ensuring seamless transitions of care for patients Qualifications Active RN or LPN license in Washington State Proven experience in managed care environments with familiarity in medical coding (ICD-10, CPT), utilization review, and clinical documentation improvement Strong knowledge of skilled nursing operations Experience with EMR Familiarity with Medicare regulations and discharge planning procedures Excellent communication skills for collaborating across multidisciplinary teams and educating staff on documentation requirements Ability to manage multiple priorities efficiently while maintaining attention to detail in a fast-paced environment Join our team to make a meaningful impact on patient outcomes through precise documentation and innovative clinical management.
