We are seeking a dedicated Licensed Practical Nurse (LPN) to join our healthcare team, providing high-quality patient care across diverse clinical settings. The ideal candidate will possess a strong foundation in nursing practices, with experience spanning acute, long-term, and community-based care environments. The LPN will be responsible for delivering safe, effective nursing interventions, supporting patient assessments, and ensuring compliance with healthcare standards and regulations.

Submission Requirements:

RN state license required BLS certificate Duties Perform comprehensive patient assessments, including vital signs, physical examinations, and health histories to inform care plans. Administer medications and injections accurately following prescribed protocols while monitoring for adverse reactions. Assist with medical procedures such as catheterization, tube feeding, venipuncture, and sterile dressing changes. Manage IV infusions and ventilator support in accordance with established clinical guidelines. Document all patient interactions thoroughly within Electronic Health Record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations. Provide wound care, including suturing and dressing changes, adhering to aseptic techniques. Support discharge planning and coordinate care transitions for patients in various settings including hospitals, nursing homes, assisted living facilities, and home care. Conduct patient education on medication management, disease processes like dementia or Alzheimer's disease, and health coaching initiatives. Participate in infection control practices and ensure adherence to safety protocols across all patient interactions.
