Job Description

FT, 80 hrs/pp, 1.0 FTE, days, varies as scheduled.
Job Summary:
The Population Health Registered Nurse (RN) functions within the nursing scope of practice under the direction and supervision of the Provider (physician, PA or ARNP) and is responsible for assisting in providing care and implementing delegated activities of care. An understanding of contemporary nursing principles and skills must be demonstrated, unique to their clinical assignment.
Family Medicine and Internal Medicine:
Family Medicine/Internal Medicine Departments:
Supports population health initiatives focusing on wellness, prevention and chronic care management. Collaborates, communicates and coordinates with the Provider and other health care team members in planning and implementing patient care. Understands and has knowledge of Medicare documentation, billing and reporting requirements associated with Annual Wellness Visits (AWV), Chronic Care Management (CCM) and Transitional Care Management (TCM) services to include patient calls and calculation of billable time. Performs face to face visits such as Medicare Annual Wellness Visits. Conducts injection clinics as appropriate (e.g. allergy injections, INR). Performs testing as directed by a provider (e.g. EKG) Helps providers close patient care gaps (e.g. colonoscopies, mammography, A1c, etc.) Education and/or
Experience Requirements:
Graduate of an accredited school of Nursing. Valid license as RN in the State of Washington. Current AHA Healthcare Provider training (BLS) Previous experience in patient care setting preferred. Prefer medical/surgical clinical experience Licensure & Certifications Requirements Must maintain
BLS Pay Range:
$41.63-$77.01
