Job Summary:
We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to coordinate patient care and ensure effective discharge planning.

The ideal candidate will have a strong background in acute care settings and proven experience in care coordination.
Key Responsibilities:
Coordinate and manage patient care plans across the continuum of carePerform comprehensive patient assessments in an acute care settingDevelop and implement discharge plans to ensure safe and timely transitionsCollaborate with physicians, nurses, and interdisciplinary teams to optimize patient outcomesFacilitate care coordination, including referrals and follow-upsMonitor patient progress and adjust care plans as neededEnsure compliance with hospital policies, regulatory standards, and documentation requirementsEducate patients and families on post-discharge care and available resources
Requirements:
Active RN license (state-specific or compact)Minimum experience inacute care settingProven experience indischarge planning and care coordinationStrong communication and organizational skillsAbility to work collaboratively in a fast-paced environment
