$1800.00•$2700.00 POSITION SUMMARY•RN Case ManagerPOSITION DUTIES
•Coordinate patient care from admission through discharge, ensuring that treatments, services, and resources align with each patient's medical needs and recovery goals. Collaborate closely with physicians, nurses, and external providers to remove barriers to care, streamline transitions, and support safe, efficient discharge planning.MINIMUM REQUIRED QUALIFICATIONS
•BSN WA/compact RN license 5 years acute case management experience 2+ years minimum of acute care hospital experience Prior travel experience Case Management and Discharge Planning, strong assessment skills, ability to discharge patients including those with difficult social circumstances such as homelessness to the next location or level of care such as home health, SNF, IP, LTAC, shelter, Assisted LivingPREFERRED QUALIFICATIONS
