A facility near La Crosse, WI is seeking a Hospice Nurse Coordinator to join their hospice and palliative care team!
Hospice Nurse Coordinator Opportunity:
Full-time, permanent position
Schedule:
Day Shift; Mon-Fri Signing Bonus available! Hospice Nurse Coordinator oversees all aspects of hospice patient care, collaborating with the interdisciplinary team to develop, implement, evaluate, and adjust individualized care plans while ensuring quality symptom management, regulatory compliance, and continuity of care across settings. Provides clinical leadership and supervision for hospice staff and volunteers, including staffing, training, performance management, quality assurance, admissions coordination, patient/family education, and maintaining positive relationships with patients, families, physicians, facilities, and community partners.
Hospice Nurse Coordinator Qualifications:
RN license with the privilege to practice in the state of Wisconsin 2 or more years of clinical nursing experience required Experience in Hospice preferred Experience in management preferred About the
