Field Registered Nurse (RN) - Downtown Area PRN Position About the Role WellSpring provides in-home skilled nursing and supportive care services for older adults and medically complex clients. We are seeking dependable field nurses to provide scheduled in-home visits based on client needs. This role is ideal for nurses looking for: Flexible hourly work Independent field work One-on-one client interaction Variety in clinical work Responsibilities Vital signs, symptom assessment, and clinical documentation Caregiver and client education and coaching Skilled nursing services, including (but not limited to): Ostomy care, Foley catheter care, Feeding tube management and site care, Wound assessment and dressing changes, etc Medication reconciliation, education, and monitoring Chronic disease monitoring (CHF, COPD, diabetes, dementia, Parkinson's) Post-discharge stabilization and readmission prevention Physician communication and care coordination Qualifications Active Wisconsin RN license Strong clinical and assessment skills Comfortable working independently in clients' homes Strong communication and documentation skills Home health, hospice, geriatrics, or skilled nursing experience preferred Compensation & Benefits Competitive hourly pay ($60/hr) Flexible scheduling based on nurse availability Supportive team environment Opportunity to grow with a rapidly expanding company To learn more, contact: raisa@wellspringcaremanagement.com
