JOB DETAILS
Travel RN-Case Manager jobs in West Allis, WI let you work in a city with lively neighborhoods and easy access to Milwaukee's amenities. You will coordinate patient care, assess needs, develop care plans, and support patients in their homes while documenting in electronic medical record (EMR) systems at the facility. The facility is a nonprofit home health organization with a supportive team culture and a wide range of services including hospice, palliative care, home medical equipment, and in-home health monitoring. Required qualifications include graduation from an accredited nursing program, an active RN license, Basic Life Support (BLS) certification, and at least 1 year of recent home health or case management experience. Recommended skills include strong communication, organizational ability, and proficiency with EMR systems.
