(includes, but not limited to) Make a true difference—one patient, one family, one compassionate moment at a time. Develop and implement individualized care plans based on current and prior medical history Deliver attentive, one-on-one support to patients and their families, ensuring comfort, dignity, and understanding Conduct thorough assessments of physical, emotional, and spiritual needs throughout the care journey Accurately monitor and document vital signs including temperature, pulse, respiration, and blood pressure Administer physician-ordered medications and treatments with skill and empathy Educate patients and their families on care techniques and symptom management Track and communicate changes in patient condition, ensuring timely and effective responses Keep detailed and up-to-date clinical notes to reflect patient progress and quality of care provided Collaborate with interdisciplinary teams to ensure comprehensive care delivery And contribute to other supportive duties as part of a compassionate, mission-driven team And additional duties as assigned What You'll Bring toThe Team:
